Looking for others who work in the value-based space (I work in administration at a health system). I have seen the following pros and cons to VBPs, and am curious what others’ experiences have been.
Pros: - Attempt to change incentives. I see recognition on both provider and insurance sides that fee-for-service is broken and needs to change. It’s refreshing to hear insurance executives say this out loud in meetings. - Meeting topics and goals shift to actual patient value, away from “productivity” measures like RVUs and towards quality measures and reducing total cost of care. - Beginning to focus on social determinants of health - housing, food access, mental/behavioral health, etc.
Cons: - VBPs are still built on top of fee-for-service, not as a replacement to it. Therefore, there are still plenty of volume-based incentive metrics like # of screenings and visits. - Not yet transformational. At my health system VBPs are only like 1-2% of total revenues, and we’re about 5-6 years in. - “Quality” is still, IMO, incorrectly defined as cancer screenings and annual wellness visits…see above on volume-based. Instead of volume of cancer screenings…how about measuring improvements in early detection rate and outcomes of your cancer treatments program(s)? - Risk-capture and coding/documentation are keys to success in all the VBPs I’ve seen. This means more clinical time at the keyboard (more fee for service activity!!). This is a heavy push from insurance, since their premiums go up if risk scores go up.

I made these completely from scratch! The vases are made from dollar store candle holders and 25¢ plastic cups. I glued them together and then spray painted them black. Then I used styrofoam and fake flowers to create the flower balls on top! Altogether about $11 a piece to make. And I think they came out gorgeous 😍💜

The last five or so years have been a wild ride for me. For a few years out of high-school I was obese. I'm in premature menopause (currently 25 y/o, was diagnosed at 13) and doctors indicated that this has a pretty drastic effect on my metabolism as well. It took me two years, but I was able to drop down to about 115 lbs (I'm 5'3") and I felt.. normal? I was obsessing and restricting constantly, but I felt good about it, almost.
Then, about a year ago I guess I hit a depressive period but everyone, especially my husband, was "so proud of my recovery journey." I began obsessing over food in a completely different way and just never felt full again, even if I made myself sick. I've gained so much weight (currently 200 lbs AGAIN) and I feel like I'm losing my damn mind. We just moved to a new part of town and I started a new (wonderful) job where I've been finding it difficult to make friends, and on top of that my husband is extremely protective about my struggle with food and freaks out a little bit if he finds me falling back into old patterns. I feel like I don't even have anyone to talk to or hopefully offer support. I feel alone. Does anyone have advice? I honestly have no idea what to do.

Hello everyone, I was doing a comparison between search platforms, and the results I obtained when using Solr versus Elastic Search were very different.
The exercise is the following: I have 450k documents and 848 answers pertaining to frequently asked questions (FAQs) both in portuguese (PT) . In the first iteration I index 50k documents plus the 848 answers of the FAQs. Then using the 848 questions from the FAQs I query Solr utilizing BM25. I then check the accuracy of the returned result by verifying if the ID of each FAQ exists in the top K within the retrieved IDs from the search query. For each following iteration I add 50k new documents and repeat the querying process and check the accuracy. This process was done the same way using Elastic Search.

How I setup Solr

First I started by creating a docker containter for Solr version 9.2.1 which went successfully and allowed me to access the Solr Admin UI. Through the docker cli I created a core through the command bin/solr create -c solr_bm25 Then utilzing the Solr Admin UI, selecting the "solr_bm25" core I went to the schema panel and there I added a Field named "text" with the field type "text_pt" which comes already with Solr.
I then ran the code where I index and query Solr at each iteration. This next code is only the essential code I'm using for indexing and querying Solr ``` import pysolr import urllib.parse
